Our Provider’s goal is to provide the best possible treatment to our patients. For all new patients please have your medical records sent to our office, if possible, two weeks, before your appointment. The doctors try to review all records before your appointment. For all established patients, if you go to any specialist office, have any types of X-rays, Scans, Ultrasounds, Procedures, Surgeries or any other notes that we as your Primary Care should have, please fax the records to 877-254-0566.
We have attached our Medical Records request form below. Please make sure that you write the Complete Physician /Specialist/ Facility name and phone number that you are requesting the information from. Please make sure that your name and date of birth are legible, and you sign and date the form.
Patients of Doctor Feeney: We have a view only copy of some records from Temecula Valley Family Medicine. Unless you sign a Medical Release form, we will not be able to request a faxed copy from Dr. Boggeln’s office. For any records before 1/16/2017 please sign a medical release form or contact Dr. Boggeln’s office at 951-489-8396 or fax a request to 951-848-9500.
If you have moved or need a copy of your medical records, we do charge a $25.00 fee for printed copies regardless of the size of the chart. There is no charge to fax or email medical records to a physician or specialist office, but a signed medical release form will be required.
For extraction of medical records for Law Offices there will be a $25.00 fee require.

Dr. Feeney has a strict New Patient Policy. If he has never seen a patient, they need to complete the medical records form above and email it to jharris@atigafamilypractice.com or fax it to 877-254-0566. He expects to have 2 years of medical records received by our office within 5 business days of the scheduled New Patient Appointment. If you are having trouble with medical records being transferred to our office or have not been seen by a doctor in the past two years, please answer in detail Dr. Feeney’s Medical History form attached below.
